After School Extracurricular Policies Agreement
NAME OF PARTICIPANT:
1. Program Overview
Program Description: The CN Taekwondo Olympic Academy After-School Program provides structured Taekwondo training including physical conditioning, technique practice, and character development activities.
2. Payment Policy
Tuition: Tuition is due at the beginning of each semester. If a student joins after the program begins, tuition will be prorated accordingly.
Fees: Additional fees may apply for uniforms and belt testing.
3. Refund Policy
Refunds: Refunds are not allowed after attending four classes. If a student withdraws before attending four classes, a prorated refund will be provided based on the number of classes attended.
4. Make-Up Classes
Make-Up Classes: If a student misses a class, they can make up the missed class at our studio located in Plano, Texas. There are no refunds for missed classes.
5. Behavior Expectations & Disciplinary Practices
Behavior Management: Disciplinary practices may include verbal warnings, exercises, exclusion from certain class activities (e.g., not participating in pad work), dismissal from class, or reflective time. Parents will be notified only if the instructor deems the behavior problematic.
6. Dress Code Policy
Attire: Students must be dressed in athletic wear or clothing that allows for free movement and must wear athletic shoes. Taekwondo uniforms will be provided as needed for the program.
7. Medical Treatment Authorization
Medical Treatment: I give the instructors, staff, and responsible adults of CN Taekwondo Olympic Academy the authority to authorize medical or other treatment for my child. This authority begins on the date signed and continues throughout the term of the program or any renewal.
8. Photo/Video Release
Media Release: I grant permission for my child's photograph or video to be taken for promotional purposes, including but not limited to the studio's website, social media platforms, and marketing materials.
9. Release of Liability
Risk Acknowledgment: I understand that Taekwondo involves physical activity, including kicking, movement, running, foot contact, leg strength, and stretching. I acknowledge that students may run into each other or experience muscle overstretching or pulls during class. I also understand that there is no direct contact (kicking or punching) with other students. All kicking, punching, and sparring practice is conducted with equipment such as kick pads, boards, and balloons. CN Taekwondo is not responsible or liable for any serious injuries that may occur.
By signing below, I acknowledge that I have read, understand, and agree to abide by the terms and conditions outlined in this membership contract.
